Telehealth Appointments & Completing SOAP Notes
This article explains how to run a telehealth evaluation in VetVantageMD and how to complete your documentation, including SOAP notes and DBQ completion.
Starting the visit
When it’s time for the appointment, you can start the visit from the Doctor Dashboard, the Patients page, or your calendar.
Marking attendance
At the start of the visit, mark the veteran as present, late, or absent. If the veteran is late or absent, the platform can notify them and support rescheduling workflows.
Telehealth controls
VetVantageMD uses a Zoom-powered telehealth session with familiar controls. During the visit, you have access to:
- Camera and microphone controls
- Chat
- Screen sharing
- Closed captions
- Background blur
Recording and transcript
After the visit, you’ll have access to the recorded video and an automatically generated transcript. This helps you review the evaluation, validate key details, and better understand the veteran’s responses during documentation.
Completing SOAP notes and DBQ’s
As you conduct the evaluation, complete the remaining sections of each DBQ. Veteran responses remain visible for clinical context, and subjective sections may already be pre-filled from PAQ’s.
Use the information gathered during the visit to document your SOAP components:
- Subjective: Confirm symptoms and functional impact reported by the veteran
- Objective: Add measurable findings and exam observations
- Assessment: Provide clinical interpretation aligned to the DBQ requirements
- Plan: Document next steps, follow-up needs, and any recommendations
Provider remarks may be auto-filled to reduce documentation time, but you should review all auto-filled content for accuracy before finalizing.
Finalizing the evaluation
Once all DBQ’s are complete, finalize the evaluation directly within VetVantageMD. The entire workflow is completed in one secure platform.
